Patient Terms

1. The relationship

AgeDefy is a technology and administrative services company. It is not a medical practice, a pharmacy or a prescriber, and it does not practise medicine or control clinical decisions. Care is provided by independent licensed clinicians. Medication is dispensed by licensed pharmacies. Your relationship for clinical purposes is with your treating provider.

2. Eligibility

You must be 18 or over and physically located in a state where a partner provider is licensed at the time of your visit. You agree to give complete and accurate health information; incomplete information can lead to unsafe recommendations.

3. Fees and billing

• Fees are charged on a direct-pay basis. We do not bill insurance.
• Prices are shown before checkout and may change with notice
• Recurring plans bill automatically until cancelled — see Cancellation & Shipping
• You authorise us and our payment processor to charge the payment method you provide

4. Medications

Prescriptions are issued only where a provider determines treatment is medically appropriate. Some preparations are compounded by state-licensed compounding pharmacies; compounded medications are not FDA-approved finished drug products and have not been evaluated by the FDA for safety, efficacy or quality. Brand names are trademarks of their respective owners.

Do not share prescribed medication with anyone. Report side effects to your provider, and report serious adverse events to the FDA MedWatch program.

5. Laboratory testing

Lab panels are ordered following provider review. Results are returned through the platform and reviewed by a provider. Lab results are one input to your care and are not a diagnosis on their own.

6. Communication

Messages sent through the platform are for non-urgent matters and are usually answered within one business day. Do not use messaging for emergencies — call 911.

7. Your responsibilities

• Keep your contact and payment details current
• Attend scheduled visits or cancel in advance
• Follow the plan your provider gives you, and tell them if anything changes
• Tell your provider about all medications and supplements you take

8. Ending the relationship

You may stop using the service at any time. A provider may end care where continuing would not be clinically appropriate, and will tell you if so. We may suspend an account for breach of these terms.

9. Records

Your medical record is held by your treating provider. Request copies from that provider; retention periods are set by state law.

10. Disputes

These terms are governed by California law. Clinical concerns should be raised first with your treating provider or the partner clinical group, which is responsible for the care delivered.
